Forms regularly used are listed below with an explanation regarding their use.
Monthly Operating Report - to be filled out at the end of every month and sent to the Melita Office.
Spare Driver Payment Voucher - Drivers must complete this form if spare drivers were required on the route during the month. This form is to be submitted to the Operations Supervisor.
Leave Request Form - Is to be completed any time the regular driver is not driving.
School Bus Conduct Report - Is used to report violations of the Students Code of Conduct. This form is to be submitted to the School Principal.
Warning Light Violation / Complaint Form - Drivers are to complete this form if a vehicle passes their school bus while they are loading / unloading. This report is to be taken to the RCMP.
Accident Report Form - This form should be submitted to the Operations Supervisor, who will then submit it to Manitoba Pupil Transportation.
Daily Inspection Report - Is for reference to remind drivers of expectations of daily inspecting.
Bus Evacuation Drill Checklist - For reference when conducting bus evaculations.
